3.2.1 Referral Process for Young Carers

1. New Cases

Duty worker in Referral and Assessment team processes referral as either:

  1. No Further Action
  2. Advice and information is given - e.g. referral back to voluntary sector
  3. Initial Assessment is carried out by the Referral and Assessment team after manager's authorisation

2. Young Carers Assessment

After completing an Initial Assessment, the following is recommended:

  1. An alert is sent, via Carefirst, to the Group Manager in the Safeguarding & Care Planning Team and to the rest of the team as a fail-safe
  2. The Group Manager (or the duty manager) then allocates the case to the Young Carers Social Worker (or the duty worker) who completes the Young Carers' Assessment within 10 working days

3. Next Steps

After completing the Young Carers' Assessment, one of the following is recommended:

  1. No Further Action
  2. Advice and information is given - e.g. referral back to voluntary sector
  3. An Initial Core Assessment completed by the Young Carers Social Worker within 35 working days

N.B. Long-term Teams

A special referral form will be devised for the long-term teams to make a referral to the Young Carers Social Worker where it is identified that there is a Young Carer within the family who may be entitled to services.  After the referral, STEP 2 onwards is completed.

A box will be added to the end of Initial Assessments which will recommend a Young Carers Assessment and can be triggered by the Duty Social Worker in the Referral and Assessment teams.  This will be active on CareFirst by 26th September 2008.
